If he is already subscribed, it should only trigger once (since there is a cookie that should block double postbacks).

In case his browser doesn't support push notifications, I think DENY option is triggered.

So I noticed on your Monetizer profile page you can enter your "Push Subscription Postback." Is this the same as firing the pixel yourself? Or is this something else?

Yes, it is the same thing.

After not getting any postbacks, I realized that the js version with your own lander does not post back. You do need to fire your own pixel.

Yes. I noticed Monetizer site says js push installation doesn’t post back.
