Anyone know a good quiz plugin/service that would help me setup a quiz where the answers can be synced to a lead profile on a database on my email marketing app ?
I am trying to build a simple one-page LP like this one:

As you can see it has questions that can help the owner market to the surveyor based on what type of credit they have (Excellent, Good, Poor) as well as tags the lead as either a home purchase lead or a refinance lead.
I want something like this so i can build my own email list for a refi offer my AM told me is doing very well.
Any thoughts on how I can go
