I like Bootstrap so I decided to take Nick's survey LP for the Survey Pops course and convert it into a simple Bootstrap template:

I used the latest version of Bootstrap and their CDN. This is what it looks like:

I suggest you make many changes and split test them all :)

Here is the code (also attached):

HTML:
<!doctype html>
<html lang="en">

<head>
     <meta charset="utf-8">
     <meta name="viewport" content="width=device-width, initial-scale=1">

     <title>OMG</title>

     <link href="https://cdn.jsdelivr.net/npm/[email protected]/dist/css/bootstrap.min.css" rel="stylesheet" integrity="sha384-1BmE4kWBq78iYhFldvKuhfTAU6auU8tT94WrHftjDbrCEXSU1oBoqyl2QvZ6jIW3" crossorigin="anonymous">

     <style>
         body  {
              background: grey;
              margin: 10px
         }

          .container {
              background: #fff;
              max-width: 300px;
          }

         h1 {
              margin-top: 10px;
              text-align:  center;
         }

         .timer  {
              color: red;
              font-weight: bold;
              font-size: 22px;
         }

         .small {
              font-size: 12px;
              color: grey;
          }
    </style>

     <!-- propush goes  here -->
    <!-- back button goes here -->
</head>

<body>
     <div class="container">

         <div  class="row">

              <div class="col-sm-12">

                   <p>
                       <img src="https://via.placeholder.com/300" width="100%" alt="OMG"  />
                  </p>

                   <!-- subheading -->
                   <p class="lead">
                       Taking this survey has changed my life. Now, it's your turn 👇
                   </p>

                   <!-- sentence to convince -->
                   <p>
